A doctor visit will be needed to obtain a physician’s statement, verifying your medical condition. Your current doctor may be willing to sign a physician’s statement, but if unable to obtain, then book an appointment with our recommended providers at Alternative Wellness.

Begin State Registration

After you’ve received your Physician’s statement, you will register for an account via Complia. There will be a $31.85 processing fee. You will need a headshot, copy of ID and physicians statement – all in electronic format.

TIP: You can either scan or take photos of documents. It’s also easier to complete application behind a computer.

How to Switch Providers

To access the form to switch providers, cardholders are required to log in and register through the state licensing system, Complia.

NOTE: If your card is not expired, and you are using Complia for the first time, the state will waive the card reprinting fee. All you need to do is create an account in Complia, upload a copy of your medical marijuana card in place of the doctor recommendation and select mail in payment. You do not need to send in a payment.
