A doctor visit will be needed to obtain a physician’s statement, verifying your medical condition. Your current doctor may be willing to sign a physician’s statement, but if unable to obtain, then book an appointment with our recommended providers at Alternative Wellness.

Begin State Registration

After you’ve received your Physician’s statement, you will register for an account via Complia. There will be a $31.85 processing fee. You will need a headshot, copy of ID and physicians statement – all in electronic format.
